All employees, contractors, volunteers and visitors of First Presbyterian Church are required to fill out the Covid-19 screening form before entering the building.

The form must be filled out once a day, on every day that you are entering the building.

Please do not fill out the form if you do not plan on entering the building today.  If you have a question on when you need to fill out the form, please call the church office.
